The D Inquiry follows the judgement of the Court of Appeal in February 2006.  You can access the Court Judgment.

Mr D received serious self-inflicted injuries while a prisoner in HMP Pentonville in December 2001.  The Court held that the circumstances required the State to conduct an Inquiry compliant with Article 2 of the European Convention on Human Rights.  The Inquiry is led by Stephen Shaw CBE, the Prisons and Probation Ombudsman (PPO).
