The 'D Inquiry' followed a judgement of the Court of Appeal in February 2006.

Mr D received serious self-inflicted injuries while a prisoner in HMP Pentonville in December 2001. The Court held that the circumstances required. The Inquiry was led by Stephen Shaw CBE, the Prisons and Probation Ombudsman.
